How to Print an Insurance Form
Summary:
A patient record may contain multiple insurance coverages such as a Primary plan, a Secondary plan and for some Tertiary or Quaternary plans. If so, you can print forms for each of these coverages.
Steps:
1. Open the patient record.
2. Click the icon on the toolbar.
The Print Forms/Submit Claims dialog appears.
3. Double-click Insurance Form.
The Select Transactions to Print window appears.
4. The next step is to select the insurance plan for which you are printing the form. By default, Primary is shown beside Select Coverage. If the patient has multiple coverages (Secondary, Tertiary, Quaternary), do the following:
a. Click the Select Coverage button.
The Insurance Coverage window appears listing each of the coverages (Primary, Secondary, Tertiary, etc.
b. Click on the insurance coverage for which you are printing a form.
c. Click OK.
Your selection is now shown beside Select Coverage.
5. Notice that each procedure for the current date or past date that has not yet been printed or submitted electronically is shown. Do one of the following:
- To print ALL procedures for a specific date, click the applicable check box in the Date column. All procedures associated with the date are automatically checked.
- To manually select the procedures you want to print, regardless of date, click the check box in the Print/Submit column beside each procedure you want to print.
6. Click OK.
The Standard Dental Insurance Form containing the procedures you selected is displayed on screen.
7. Click the Print button to send the form to the printer.
8. Click Close.
9. Click Cancel to close the Print Forms/Submit Claims dialog
